PCA / personal care aide: A worker who assists with authorized personal care. Programs may also say attendant, personal assistance, or aide. Titles are not interchangeable across every payer.

More than one lawful pathway
Virginia does not reduce home-attendant qualification to a single certificate name. 12VAC5-381-290 currently lists six alternative pathways, plus the English language requirement.
Do not oversimplify
Do not tell applicants that everyone must have exactly the same certificate. Confirm the pathway that applies to the service being delivered and to current regulation.
Language requirement in the same section
The current text requires home attendants to be able to speak, read, and write English and to meet one of the listed qualifications.
Written instructions before the first visit
For home attendants assisting with skilled services, 12VAC5-381-330 requires specific written instructions for the client's care from the responsible health professional before the initial delivery of services, and work under that professional's supervision.
Why an RN-developed plan matters for personal care
12VAC5-381-360 requires personal care to be delivered based on a written plan developed by a registered nurse in collaboration with the client and family. That is why training, competency, and staying inside the plan matter.
Annual in-service education
Home attendants providing personal care services shall receive at least 12 hours annually of inservice training and education. Inservice training may be in conjunction with on-site supervision. Home attendants assisting with skilled services have a parallel 12-hour annual requirement in 12VAC5-381-330. A DMAS CCC Plus bulletin also reminds agency-directed personal care and respite providers of a minimum of 12 documented hours of agency-provided training related to those services.
CPR is not listed as the qualification itself
12VAC5-381-290 does not say CPR certification is the home-attendant qualification. CPR may still be an Eden employment, assignment, or professional-development item. Keep those categories separate.
Eligibility and applicability
Program rules, assessments, and payer authorization decide what can start. Medicaid enrollment, waiver enrollment, service eligibility, service authorization, and authorized hours are not the same status.
Assessment and documentation
Depending on the program and service, an assessment and supporting documentation may be required. A nurse does not automatically visit first for every request.
Authorization and approval
When authorization is required, the payer or MCO reviews the requested service and hours. Outcomes can include approval, partial approval, a request for more information, or denial. Eden does not approve the benefit.
Hours and limits
Requested, assessed, authorized, scheduled, and EVV-verified hours can all differ. This site does not invent a universal hour number.
How services can interact
Personal care, respite, companion, adult day, and transportation follow separate rules. A plan may authorize one, more than one, or none.
Family caregiver implications
A parent or spouse is not automatically a paid caregiver. Virginia LRI rules apply only in defined relationships and only when the current official policy and authorization allow it.
Agency-directed vs consumer-directed
Agency-directed: a licensed agency is the employer. Consumer-directed: the member or representative is the employer of record, with services-facilitator support where the waiver allows. Eden's typical public role is agency-directed care.
Insurance and MCO implications
Managed-care members usually follow their plan's authorization and appeal process. Fee-for-service members follow DMAS processes. A plan existing on the official roster does not mean Eden is in-network.

What training does a PCA need?
Virginia licensed home-care organizations must use one of the qualification pathways in 12VAC5-381-290. That can include nursing education, Board of Nursing nurse-aide education or certification, a qualifying nursing-student pathway, a competency evaluation meeting the cited federal criteria, or — for personal-care attendants only — the DMAS Personal Care Aide Training Curriculum. Agencies also provide orientation, competency, supervision, and at least 12 hours of annual inservice education for personal-care home attendants.
Does everyone need a CNA?
No. Nurse-aide certification is one listed pathway, not the only one. Other pathways include nursing education, a Board-approved nurse-aide education program, a qualifying nursing-student pathway, competency evaluation, or the DMAS personal-care curriculum for personal-care attendants.
